How we obtain your personal data: 

You provide us with personal data either by phone, email or in person, with qualified or reception staff. This includes your name, address, date of birth, telephone numbers and email address, as well as NHS and National Insurance numbers. It may also include details of your health and your family’s health.

How we use your personal data: 

We use your personal data in the provision of your eye care examinations and optical appliances. We also use this data to manage the reminder system for your future eye examinations and maybe to arrange onward referral to other health care providers (both private and within the NHS).

We use post, telephone, email and SMS messaging for the purpose of arranging appointments, discussing any issues, for informing you of the status of any eye-care appliances ordered, or when your next appointment is due. We do not routinely use these methods for marketing purposes but will seek your express verbal permission to contact you for marketing purposes.

Sharing information: 

To ensure complete care relating to the outcome of your eye examination, we will from time to time need to share your personal data with other healthcare providers (including other Optometric practices, as well as your GP and NHS or private hospital departments). This will only be done with your permission and in your knowledge.

Your Individual Data rights:

  • Subject Access Requests:
    The General Data Protection Regulation (GDPR) grants you the right to access particular personal data that we hold about you. This is referred to as a subject access request. We will respond promptly (the GDPR requires us to reply within one month of this request).  Our response will include all personal identifiable information that we hold about you eg name and address, as well as the purposes for processing this information and any persons or entities that we have shared this information with.
  • Right to Rectification:
    You have the right to have your data rectified, or corrected, if the personal data we hold about you, is incorrect or incomplete.
  • Right to Erasure:
    You have the right to obtain from us the erasure of personal data concerning you without delay, excepting where we as healthcare providers have a duty to hold this personal data for medical or eye care reasons (usually up to ten years after an eye examination).
